The smaller syringe needles will pierce in and out fairly easy with a relatively long lifespan; hwr, larger bore needles tend to open gaps unless inserted in a specific manner.
In order to lengthen time used on the large bore needles as well as the smaller ones is to follow the piercing tip of the syringe's needle. The sharp point is slanted at an edge w/a hollow opening for the metal needle bore. If the needle is pierced through the cap with the sharp point at a slight angle and the hollow opening upward, this should give more or rather a longer lifespan when drawing fluids through the rubber caps. It is possible to flip the rubber caps to get more use out of them although as long as you pierce the top slowly and carefully while ensuring you are piercing it at a slight angle, this should lengthen the time-frame of use with little to no tearing of the rubber grommet inside the cap.
Again, pierce at a slight angle with sharpest point going into the cap and bore hole opening turned upward as the needle pierces then draw what is needed out and gently remove the syringe.
These caps with rubber grommets do not necessariliy require kid gloves but to ensure the longest use of each cap with grommet, a little finesse goes a long way. Also, it is possible to flip the grommet to use the other side as long as there are no obvious holes that will cause leakage.
These piercable scsrew caps are an amazing addition to the e-liquid options. With practice, it will become much easier with less destruction to the grommet. This is a lot like piercing diabetic vials and drawing out insulin. It isn't easy but over time you get the hang of it and it becomes second nature; hwr, take special care with larger bore syringe needles.
*Not for use with blunt needle syringes; although, if using a large bore needle due to the thickness of the e-liquid, it is possible to draw e-liquid from the bottle then when the syringe holds the amount desired, unscrew the piercing needle and change out to a blunt needle to use when filling cartos or other like parts serving as an atomiser for the e-cigarettes.

*It does take time to learn to piercing the grommet correctly thus adding more to the lifespan of said grommet but it is possible.

This type of lid w/the piercing top is something I have spent the last 30+years using. My earliest usage was for drawing vaccines for animals we had on a farm during my youth. The longest time I spent using this kind of piercing top has been in the area of drawing up insulin of multiple types and multiple required insulin types for my husband and my late grandfather (he died from natural causes in his early 90s). This is very similar and works identical to the insulin and vaccine tops of the vials containing the medication. It's easy to use, prevents excess oxygen as in open bottles, and it prevents spilling your precious e-liquid in addition to the cost of what you need to pay to replenish your e-liquid supply.
A short primer to using these types of tops includes a few steps and it will depend upon how large of a syringe you will be using so please take note when you are pulling your "air-shot" before drawing the e-liquid. In order to maintain cleanliness, always start with a syringe and plunger that is clean & is dry.
1: Before injecting the needle into the top, pull the plunger to the syringe back to approximately 3 to 5 units, again, depending upon your syringe size. If the syringe is smaller, use a smaller air-draw. Larger syringes for filling the new super-mega cartos may require a little larger air-draw.
2: Inject the needle of the syringe into the rubber material, preferably in the center. Press the air-shot into the bottle.
3: Without removing the needle, turn the bottle upside downside with the bottom of your bottle at the top and the plunger of the syringe at the bottom.
4: Pull the plunger of the syringe down to draw the e-liquid into the plastic area of the syringe until reaching the approximate amount necessary to fill your cartomizer or tank that requires e-liquid. If you draw too much out, simply push the plunger up and this will insert the e-liquid back into the bottle without any spillage.
5: Remove the needle of the syringe from the lid then turn the bottle back upright - if desired, you can flip the bottle back upright before removing the needle or even do so with the bottle in your hand sideways. Either way, it won't leak because of the material used in this type of top. It seals and reseals when punctured and when the needle is removed; however, please note that over time and wear, you will need to replace the top as the rubber material will begin to breakdown over time.
6: Once the needle is out of the bottle, you can set the bottle aside then check the syringe for air. If you find an air bubble, you can draw back the plunger a small bit then light thump the syringe with your fingers on your free hand to bring the bubbles to the top where the needle is attached. Sometimes, you might need to pull the plunger down a bit more to accommodate the air inside then thump the syringe again to ensure the air as at the top where you can easily press the plunger up very slowly until there are no air bubbles in the liquid.*
7: Insert syringe into your cartomizer, tank, or other e-liquid holding chamber making sure not to overfill. Any remaining e-liquid in the syringe can be left inside if you cap the needle then put it away and out of reach of children and/or pets. You can also reinsert the needle and press the contents back into the bottle although, for sanitary reasons, I recommend filling an additional cartomizer as a spare when the initial one runs out or holding the liquid in the syringe to top off the carto when needed or after a few minutes or so of use in order to keep the e-liquid fresh.
* There is no need to ensure all air bubbles are removed as this is not like pulling or drawing up medicine such as insulin. It is simply a second nature habit, if you will, that comes with drawing up vaccines for farm and domesticated animals and some 28+ yrs of drawing up insulin, which has to be free of air as it can cause major health risks if not removed in it's entirety.
** In order to be as sanitary as possible, wipe the top of the lid with a wet washcloth or towelette to ensure the lid is clean before inserting the needle and after removing it. Again, this is for sanitary reasons. In addition, rinse out and clean the syringe and needle with warm soapy water or grain alcohol (ingestable alcohol) to keep your syringe and needle clean.
